Tara NuzzoGeriatric Care Manager

From a young age, I realized I had unique talents for both compassion and keen observation. These skills allowed me to notice things that others often overlooked, frequently enabling me to be of help to those in need. This attention to detail helped me to excel in learning new skills while assisting others in doing the same.

When I was seven years old, I joined my first swim team and quickly excelled, even qualifying for the junior Olympics! After many years of great success, at the age of 15, my swimming career halted due to two major shoulder injuries which impacted my ability to swim. I chose to undergo surgeries on both shoulders, hoping to swim again. This pivotal decision shaped my path and ultimately led me to becoming an occupational therapist.

It was during my recovery, where an incredible team of therapists and doctors supported me, that I was able to overcome one of my life’s biggest challenges. I discovered this medical team’s devotion to helping others, and immediately felt an overwhelming calling to dedicate my life to that extraordinary cause, as well.

Since earning my Masters in Occupational Therapy, I’ve had the pleasure of assisting patients of all ages; pediatrics to geriatrics. I’ve worked in skilled nursing facilities, rehabilitation centers, and even the homes of my clients. I have consistently strived to provide the highest level of care. My experience and passion truly lies in treating adults, older adults, and adults with dementia.

As a Certified Dementia Practitioner, I have found new and innovative ways to enhance my clinical and communication skills. As a therapist with experience in large facilities, I have seen how patients can sometimes feel abandoned during complex medical situations. I understand the crucial importance of having an advocate and a strong, positive support system. With this experience and ability to make an impact, I found a love for care management.

In my role as a Geriatric Care Manager with Peace Aging Care Experts, I am excited to use my knowledge, experience, and passion in assisting my clients and helping them and their families navigate their healthcare journey, while providing them with the best possible support and care.
