Geriatric Care Manager (Per Diem)

Qualifications

  • Required: Registered Nurse (RN) or Licensed Social Worker (LSW)
  • Required: Caring individual who enjoys helping clients and their families
  • Preferred, Not Required: Certified Case Manager (CCM) or Care Manager Certified (CMC)

Job Responsibilities

  • Meet with our new clients to conduct initial assessments (determine medical, social, emotional needs)
  • Create a Plan of Care for our clients’ current and future needs
  • Build a relationship with our clients to become a trusted member of their care team
  • Manage our clients’ care ongoing (with a focus on their medical, emotional and social needs)
  • Communicate regularly with our clients’ families and/or responsible parties
  • Schedule & attend our clients’ medical appointments
  • Manage client medications & ensure refills are available

About the Working Location

Our care managers operate out of their home offices.  Peace Aging Care Experts’ service area covers the following counties: Morris, Bergen, Hudson, Essex, Union, Middlesex, Passaic & Somerset.  We assign clients to care managers based on the clients’ needs and location to ensure that care managers are not driving unreasonable distances for visits.

About “Per Diem”

At Peace Aging Care Experts, our care managers work “per diem,” meaning they are only paid for the time they work (this includes making/answering phone calls, emails, research, driving to clients, visiting with clients, etc.).  Care Managers typically work 15-30 hours depending on their preference, the current client base, and clients’ needs (which do fluctuate).  Because our clients count on our responsiveness and ability to handle situations quickly, it is best if this job is the only position the candidate holds.

Administrative Assistant (Part-time)

Qualifications

  • High school diploma/GED
  • Positive-minded and caring individual who enjoys interacting with clients and staff
  • Dependable, strong work ethic
  • Proficient in Microsoft Office Suite
  • Excellent typing skills
  • High attention to detail
  • Well-organized
  • Ability to maintain client confidentiality

Job Responsibilities

  • Answer phones, respond to emails
  • Provide administrative support to the Care Management and Recreational Therapy team
  • Assist with office errands and ordering of supplies
  • Manage client online grocery orders
  • Data entry
  • Scan and archive client documents

About the Position

The Administrative Assistant is an integral part of our dynamic Peace Aging Care Team! In this role, you will work independently and as a team, providing high-quality services to our clients and supporting our care management and recreational therapy staff. This position is part-time, 20 hours per week (4 hours per day, 5 days per week).
