Yael Rotter-Salama, OTR/L, MS, CAPS
Geriatric Care Manager
I entered the business world out of college and remained there for several years until volunteering in the rehabilitation department of a hospital. The moment I walked into this healthcare setting, I immediately knew caring for people was my calling. I left the corporate world and went back to school to obtain my Master’s degree in occupational therapy. Now with two decades of experience working in hospitals, rehabilitation centers, and home care, I have had the privilege to help individuals of all ages enhance their quality of life and live at their highest potential. Throughout my career, I have worked closely with the geriatric population, developing individualized care plans, conducting assessments, and coordinating multidisciplinary services. My extensive hands-on experience has equipped me with a thorough understanding of the complexities involved in geriatric care, including physical, cognitive, and emotional considerations. I am a Certified Age in Place Specialist and possess first-hand knowledge of how to facilitate living safely and independently for as long as possible. On a more personal level, I have coordinated the care of both my parents through illness and eventually hospice. I truly understand the importance of having a well-equipped, knowledgeable professional for support during the most stressful of times. I feel passionate about contributing my knowledge and skills to supporting others.
