Cindy Hand, PT
Geriatric Care Manager
From an early age, I was drawn to a career focused on helping others, particularly within the healthcare field. Although I initially considered nursing, I ultimately earned a Bachelor of Science in Physical Therapy and found my true passion in patient care—especially working with the geriatric population.
This field offered the ideal balance of building meaningful relationships with patients while actively supporting their recovery. I found it incredibly rewarding to identify goals with my patients and support them through each step of their journey. Most of my career has been spent in inpatient, sub-acute, and skilled nursing settings, where I’ve had the privilege of collaborating with patients and their families to enhance quality of life and make a meaningful impact in their daily lives.
My experience as a Physical Therapist naturally led to a transition into Care Coordination, where I managed post-acute care members in rehabilitation settings. In this role, I worked closely with interdisciplinary teams to advocate for safe and effective discharge plans, ensuring smooth transitions across levels of care. I found great fulfillment in guiding members through the healthcare system and providing education to support their recovery and well-being, but as much as I enjoyed this position I realized how much I missed the day to day interactions with my clients and having that personal relationship.
Peace Aging Care Experts was the logical next step as it is a balance between both worlds. I get to use my experience as a Physical Therapist and a care coordinator to be an advocate for the geriatric population and their caregivers. I am so excited to be a part of this team.
